Laparoscopy-assisted Total Gastrectomy for Metastatic Lung Cancer of Remnant Stomach

Abstract

|
We report a case of the successful laparoscopy-assisted total gastrectomy for metastatic lung cancer of remnant stomach. A 66-year-old male visited to our hospital with chief complaint of nausea and indigestion. He had undergone a radical subtotal gastrectomy with retrocolic Billroth II anastomosis due to early gastric cancer 15 years ago. He also underwent left upper lobectomy of lung because of squamous cell carcinoma 2 years ago. Gastroscopic finding revealed Borrmann type II mass of the remnant stomach and biopsy proved the lesion as metastatic squamous cell carcinoma. On preoperative FDG-PET and CT scan, there was no other metastatic lesion. After five ports were placed, careful adhesiolysis was continued around stomach. Distal esophagus, remnant stomach, and gastrojejunostomy site were clearly exposed. We finally performed laparoscopy-assisted total gastrectomy and Roux-en-Y esophagojejunostomy. The patient started to eat semi-fluid diet on postoperative 3rd day and discharged on 7th day. In conclusion, laparosopic procedure in remnant stomach is technically feasible in selected cases and long term follow up is necessary for evaluation of survival and quality of life.
